Job summary

Aviata Health Group is seeking an experienced In-House Infectious Disease Nurse Practitioner to lead care at our geriatric facilities in Florida. You will perform comprehensive exams, prescribe treatments, and coordinate care with physicians and the broader care team.

Qualifications

• Active Florida Nurse Practitioner License (unencumbered).
• Master’s Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Responsibilities

• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care
