Job summary

Aviata Health Group is seeking an In-House Nurse Practitioner specializing in Infectious Disease to lead care in our skilled nursing and long-term care facilities. You will build relationships with residents, families, and staff while guiding clinical care and policy development.

The role emphasizes proactive, patient-centered infectious disease management, with no night or weekend requirements and opportunities for bonus performance along with CME support.

Qualifications

• Active Florida Nurse Practitioner License (unencumbered).
• Master’s Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience in skilled nursing/long-term care settings.
• Ability to engage and communicate with elderly and chronically ill patients and families.
• Meet a minimum of 12 patient visits per day; bonuses for surpassing this target.
• Knowledge of Medicare/Medicaid and Health Plan benefit structures is a plus.

Responsibilities

• Performing comprehensive physical examinations and evaluating changes in patient conditions.
• Prescribing medications and treatments as needed.
• Developing patient care policies and facilitating referrals to specialists.
• Coordinating advanced care planning and discharge visits.
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups.
• Supporting the physician-patient relationship and addressing gaps in care.
• Provide essential care and leadership within the facility.
