Job summary

Aviata Health Group is seeking an In‑House Nurse Practitioner specializing in Infectious Disease to lead care in skilled nursing and long‑term care facilities. You will provide clinical leadership, examine residents, prescribe treatments, and coordinate referrals with the interdisciplinary team.

The role emphasizes proactive care, collaboration with facility staff, and delivering high‑quality service while managing a caseload of geriatric patients.

Qualifications

• Active Florida Nurse Practitioner license (unencumbered).
• Master’s Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience preferred with skilled nursing/long-term care.
• Ability to engage effectively with elderly patients and families.
• Minimum of 12 patient visits per day; bonuses for exceeding target.
• Knowledge of Medicare/Medicaid and Health Plan benefit structures a plus.

Responsibilities

• Perform comprehensive physical examinations and evaluate condition changes.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely communication and documentation of assessments and follow-ups.
• Support physician-patient relationship and address gaps in care.
