Job summary

BMS Family Health & Wellness Centers in Brooklyn seeks a Nurse Practitioner - Infectious Diseases to deliver primary and specialized care focused on HIV/AIDS, hepatitis B/C, and STIs. You will work within a multidisciplinary team and manage ART, PrEP, and PrEP therapy, while advancing health equity for underserved populations.

Reporting to the Medical Director - Infectious Diseases, you will integrate evidence-based practices, maintain records, and participate in quality improvement initiatives

Qualifications

• Master's degree in nursing (MSN) or Doctor of Nursing Practice (DNP).
• Current, unrestricted New York State Nurse Practitioner certification and RN license.
• National certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
• Current AAHIVS certification
• Current DEA registration
• Current BLS certification
• Minimum two years of relevant clinical experience.
• Experience in HIV/AIDS, hepatitis management, infectious diseases, or primary care strongly preferred.

Responsibilities

• Provide comprehensive primary care, including exams, preventive screenings and chronic disease management.
• Diagnose, treat, and manage HIV/AIDS, hepatitis B and C, STIs, and other infectious diseases.
• Initiate and manage ART, PrEP, PEP, and other regimens.
• Manage chronic conditions such as diabetes, hypertension, asthma, and cardiovascular disease.
• Order and interpret diagnostic tests; prescribe medications and create treatment plans.
• Educate patients on prevention, adherence and healthy choices.
• Collaborate with physicians, behavioral health, care coordinators, and clinical team members.
• Maintain electronic health records and support quality improvement.
