Qualifications

• Unrestricted Advance Practice License in the state of practice.
• Minimum of 2 years’ experience as an Advanced Practice Provider.
• Experience in population health management preferred.

Responsibilities

• Provide care for people living with HIV and chronic diseases.
• Deliver comprehensive outpatient care consultation.
• Develop strong transitional care plans.
