Job summary

Fairfield Senior Living & Rehabilitation in Fairfield, IL is seeking a dedicated Infection Preventionist who is a licensed RN or LPN. You will develop, implement, and monitor the facility's Infection Prevention and Control Program to protect residents, staff, and visitors in alignment with CMS, CDC, OSHA, and IDPH guidelines.

Qualifications

• Current and active RN or LPN license in Illinois.
• Infection Preventionist training or CMS-compliant certification.
• Experience in long-term care or infection prevention preferred.

Responsibilities

• Develop, implement, and oversee IPCP in line with CMS, CDC, OSHA, and IDPH guidelines.
• Conduct ongoing infection surveillance, tracking, and analysis to identify trends and outbreaks.
• Monitor residents for signs of infection and ensure documentation and follow-up.
• Maintain infection control logs, surveillance reports, and required records.
• Investigate outbreaks and coordinate prevention and containment measures.
• Ensure proper standard and transmission-based precautions, PPE usage.
• Provide infection prevention education and training to staff.
• Conduct routine audits and implement corrective actions for deficiencies.
• Collaborate with nursing and medical teams on antimicrobial stewardship.
• Monitor immunization programs and outbreak reporting to IDPH.
