Job summary

Mohawk Meadows Rehabilitation & Nursing Center in Lafayette, NJ is seeking a full-time Infection Preventionist (RN or LPN) to lead our infection prevention and control program. You’ll safeguard residents and staff by building, monitoring, and strengthening our infection prevention practices in line with CDC, CMS, and NJ DOH standards.

This is a managerial position reporting directly to facility leadership, with real ownership over a program central to resident safety and survey readiness.

Qualifications

• Active New Jersey RN or LPN license is required.
• Experience in long-term care or skilled nursing preferred, with exposure to infection control/surveillance.
• Completion of or willingness to complete specialized infection prevention training (e.g., CDC/CMS IP course); APIC coursework a plus.
• Strong organizational, communication, and leadership skills are essential.

Responsibilities

• Lead the facility's infection prevention and control program and serve on the Infection Prevention and Control Committee.
• Develop and maintain infection prevention policies, procedures, and staff training curricula based on best practices and clinical guidance.
• Monitor implementation of policies across departments and support staff compliance.
• Conduct surveillance, track infections, and maintain documentation and reporting.
• Prepare quarterly reports on policy effectiveness for facility leadership.
• Ensure ongoing compliance with CDC, CMS, and NJ DOH requirements.
• Provide infection control education and orientation for staff and residents.

Mohawk Meadows Rehabilitation & Nursing Center, a skilled nursing facility in Lafayette, NJ, is seeking a full-time Infection Preventionist (RN or LPN) to lead our facility’s infection prevention and control program. You’ll safeguard the health of our residents and staff by building, monitoring, and continually strengthening our infection prevention practices in line with CDC, CMS, and NJ Department of Health standards.

What you’ll do:

• Lead the facility’s infection prevention and control program and serve on the Infection Prevention and Control Committee
• Develop and maintain infection prevention policies, procedures, and a staff training curriculum based on current best practices and clinical guidance
• Monitor implementation of infection prevention policies across all departments and support staff compliance
• Conduct surveillance, track and investigate infections, and maintain accurate documentation and reporting
• Prepare quarterly reports on the effectiveness of the facility’s infection prevention policies for facility leadership
• Ensure ongoing compliance with CDC, CMS (42 CFR § 483.80), and NJ DOH requirements
• Provide infection control education and orientation for staff and residents

What we’re looking for:

• Active New Jersey RN or LPN license — required
• Completion of (or willingness to complete) specialized infection prevention and control training, such as the CDC/CMS nursing home IP course; APIC coursework a plus
• Professional background in nursing or a related clinical field
• Experience in long-term care or skilled nursing preferred, ideally with exposure to infection control, surveillance, or regulatory compliance
• Strong organizational, communication, and leadership skills
