Job summary

Monterey Palms Healthcare Center is seeking a dynamic IP Nurse to join our clinical team in California. You will support our residents and SNFs with best-in-class infection prevention and antibiotic stewardship, maintaining surveillance and coordinating with physicians and the pharmacy.

The role requires an active CA LVN/RN, vaccination, and strong knowledge of long-term care infection control. The position is full-time, on-site with 8-hour shifts, on-call duties, and weekend coverage as needed.

Qualifications

• Active Licensed Vocational Nurse (LVN) in California.
• Fully vaccinated against COVID-19 including boosters.
• Knowledge of long-term care patient needs.
• Current First Aid and CPR Card.
• Antibiotic stewardship certification or infection control training.
• RN or LVN from an accredited school or related field training.
• Specialized training infection prevention and control.
• Ability to perform in a fast-paced environment.

Responsibilities

• Complies with attendance policy for reliable, punctual presence.
• Participate in Antibiotic stewardship program to promote safe antibiotic use.
• Review and monitor antibiotic usage patterns.
• Review antibiotic resistance patterns (MDROs).
• Maintain infection log and surveillance report for infections.
• Coordinate with Pharmacy consultant on antibiotic use.
• Communicate with Infection/Attending Physicians about antibiotics.
• Review antibiotic orders for indication and suitability.
• Review residents’ assessments for infection control adherence.
• Review labs flagged with MDRO results.
• Perform daily rounds to identify infection control issues.
• Provide staff in-service education on infection control.
• Conduct hand hygiene observations across departments.
• Act as Infection Control supervisor and ensure policy compliance.
• Investigate and report outbreaks.
• Manage exposures to infectious diseases.
• Complete Infection Risk assessments and screenings (TB).
• Oversee Immunization program for Flu and Pneumonia.
• Summarize data for monthly/quarterly reports.
• Maintain infection control logs and MDRO tracking.
• Review antibiotic treatment costs and orders.
• Attend QA meetings and report findings to the QAPI committee.
• Provide feedback to physicians on prescribing patterns.
• Support staff and resident education on antibiotic use and infection control.
• Other duties as assigned.
