The position emphasizes infection surveillance, regulatory compliance, care coordination, and safe transitions of care, with the RN licensure in MN and CIC certification as core requirements.

Qualifications

• Current RN licensure in Minnesota.
• CIC certification or ability to obtain CIC within three years of hire.
• Associate degree in healthcare-related field; Bachelor’s degree preferred.
• Experience in infection prevention, case management, utilization review, or discharge planning preferred.

Responsibilities

• Infection prevention surveillance, data collection/analysis, education, regulatory compliance, and implementation of strategies to reduce healthcare-associated infections.
• Coordinate hospital case management, utilization review, and discharge planning.
• Collaborate with physicians, nursing staff, therapy, patients, families, and outside agencies to promote appropriate resource use and safe transitions of care.
