Job summary

Fairview Skilled Rehab & Nursing Home seeks an experienced Infection Prevention & Control Nurse (RN) to lead the facility's infection prevention program. You will monitor infections, ensure regulatory compliance, educate staff, and implement evidence-based practices to protect residents and staff.

Responsibilities include surveillance, outbreak management, staff education, rounds, audits, and coordination with medical and nursing leadership to enhance resident safety and outcomes.

Qualifications

• Active NY State RN license in good standing.
• Experience in infection prevention in skilled nursing or long-term care.
• CIC certification preferred or willingness to obtain.
• Strong knowledge of CDC, CMS, OSHA, and NYSDOH requirements.
• Excellent communication, leadership and problem-solving skills.

Responsibilities

• Oversee the Infection Prevention & Control Program per CDC, CMS and NYS DOH.
• Conduct infection surveillance, reporting and trend analysis.
• Investigate outbreaks and implement containment measures.
• Educate staff on infection prevention, PPE, and isolation precautions.
• Perform infection control rounds and audits facility-wide.
• Monitor antibiotic stewardship initiatives.
• Ensure compliance during state surveys and inspections.
• Collaborate with medical providers and leadership to improve resident outcomes.
• Maintain documentation and required infection control reports.

Qualifications

• Certification in Infection Control (CIC) preferred or willingness to obtain.
