Job summary

PALS CHRYSALIS HEALTH CO. seeks an Individual Support Coordinator at the Mingo Day Program in Mingo Junction, OH. The role focuses on coordinating supports, documenting program activities, and liaising with the Program Director to ensure person-centered care.

The ISC will monitor incidents, supervise staff, and participate in mandatory staff meetings while maintaining professional relationships with community partners. A 4-year degree and valid driver’s license are required.

Qualifications

• Must have a college degree from an accredited university or equivalent and 2 years, or at least 5 years in the field.
• Must successfully pass the DOT physical.
• Current driver’s license with acceptable driving record.

Responsibilities

• Oversee and coordinate supports, services and activities for clients.
• Maintain and monitor program documentation, attendance, transportation and billing.
• Review unusual incident reports and ensure communication with the Program Director.
