Guardian Angel Senior Services, Inc. is seeking an Individual Support Case Manager to assist MassAbility participants.

The role focuses on developing independent functioning in self-care, health, welfare, socialization, communication, and vocational skills, to help participants establish and sustain community living. The position involves coordinating with the care team, providing access to resources, training caregivers, conducting home visits, and completing timely documentation.

Qualifications

• A degree in a Human Service-related field.
• Ability to research and identify information sources, advice, guidance and support.
• Good IT skills, including managing websites and knowledge of social/media communications
• Good listening and communication skills
• Able to produce written reports as required
• Knowledge of the Greater Boston area community support system
• Committed to the rights of seniors and disabled people to live dignified lives
• Flexible, reliable, and able to cope with emergencies
• Prior home care experience helpful

The Individual Support Case Manager will provide assistance to MassAbility participants on a regular basis to develop, maintain, and/or maximize their independent functioning in self-care, physical and emotional growth, socialization, communication, and vocational skills. They will work together to achieve objectives of improved health and welfare and to the support the ability of the participant to establish and maintain a residence and live in the community.

Responsibilities

• To work closely with the team involved in a participant's care.
• To provide resources to assist the individual in maintaining safety and independent in the community.
• To assist with training and supervision of caregivers and PCAs.
• To undertake home visits and document activity.
• To carry out clerical and administrative duties in accordance with the requirements of the service
• Provides necessary information on their own health status and experience.
• Completes required documentation accurately and on a timely basis.
• Maintain a clean, insured vehicle that has had manufacturer’s recommended scheduled services and transports client as appropriate.
• To work within the aims and objectives of the goals set in the individual service plan
• To be familiar and work within the policies and procedures of the service
