Job summary

CN Guidance & Counseling Services, Inc. is seeking a dedicated coach and care coordinator for the Project FORWARD program. You will assess participants, develop Individual Development Plans, and coordinate services with partners in the community.

You will engage with participants, conduct intakes, and maintain communication with a network of service providers to ensure goals are achieved and resources are accessible, including potential nights and weekends for outreach and workshops.

Qualifications

• Experience in a mental health related field and coordination of services.
• Bilingual English and Spanish preferred.

Responsibilities

• Conduct outreach to recruit/engage potential participants and community organizations serving the target population.
• Conduct intakes for potential participants in cooperation with Facilitator both in the office and in the community.
• Conduct initial and ongoing assessments of participants to determine strengths, needs, goals, and resources.
• Establish and maintain a relationship with participants as a consistent point of contact.
• Create and monitor each participant's Individual Development Plan to ensure goals are achieved.
• Assist with referrals and follow up to link participants to internal and external resources.
• Maintain coordination of care by communicating with team members and collateral contacts.
• Assist with QA activities and participate in CQI tasks and data collection.
• Foster an inclusive culture and ensure safety protocols are followed.
• Other duties as assigned.

Essential Duties And Responsibilities

• Conduct outreach to recruit/engage potential participants & community organizations serving the target population, presentations to community providers and government entities.
• Conduct intakes for potential participants in cooperation with Facilitator both in the office and in the community.
• Conduct initial and ongoing assessments of participants to determine strengths, needs, goals, and resources.
• Establish and maintain a relationship with participants and serve as a consistent point of contact with them through their participation in the program.
• Create and monitor each participant's Individual Development Plan to ensure all goals are achieved, and document progress as needed.
• Assist with referrals and provide follow up to make sure participants are linked to internal and external resources to best meet their needs.
• Maintain consistent and professional contact with team members and collateral contacts to ensure coordination of care and services.
• Assist Evaluation Team with research elements of the program and data collection in relation to the local and federal evaluations, including nFORM surveys.
• Complete program documentation of participants' progress in both participant record and electronic health records.
• Conduct relationship workshops using evidence-based curricula. Provide support to participants around workshop content and any barriers to engagement as needed both in the office and in the community.
• Participate in team meetings as scheduled.
• Support Data Manager with CQI tasks related to the program, including: participant data progress tracking, data sharing with the individuals to whom it applies to facilitate partnered decision-making & progress updates, work with the entire CN Guidance team to apply the Plan-Do-Study-ACT continuous quality improvement approach to address systemic, organizational challenges, complete QI Training during onboarding and participate in QI refresher training as needed.
• Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep participants informed of progress
