Job summary

Centralnassau seeks a qualified clinician to join the CN Guidance FACT Team, delivering services to individuals with severe mental illness whose needs are unmet by traditional approaches.

The role requires a Masters in Counseling or related field with LMSW/LMHC-LP/LMFT-LP/LCAT-LP, and at least one year of experience in integrated mental health and substance abuse assessment and treatment. Strong collaboration with court systems and community providers is valued.

Responsibilities

  • Lead responsibility for justice involved clients within dual-recovery and family support integration.
  • Complete required AOT, parole, probation, or court progress reports and escort to hearings.
  • Engage family, housing providers, defense attorneys, and community corrections for compliance.
  • Develop service plan goals to reduce criminogenic risk and promote prosocial living.
  • Coordinate with jail/prison reentry coordinators and support detentions reentry.
  • Coordinate, advocate and liaise with probation and parole officers.
  • Provide assessment, service planning, QA/IM processes, and coordination with systems.
  • Engage in assessments, case management, crisis intervention, and discharge planning.
  • Maintain case records and complete documentation on time per regulations.
  • Provide direct services to recipients in office or community settings; participate in on-call coverage.
  • Assist recipients in developing expectations from treatment and discharge planning.
  • Strengthen ability to understand and utilize therapy and resources after treatment ends.
  • Utilize supervision to improve practice skills and accountability.
  • Facilitate data sharing for partnered decision-making and progress updates.
  • Complete QI training during onboarding and refresher training as needed.
  • Foster an inclusive culture respecting diverse backgrounds.
  • Adhere to safety protocols to ensure a safe working environment.

Job description

JOB DEFINITION

Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.

EDUCATIONAL REQUIREMENTS

Masters Degree in Counseling or related field, with LMSW, LMHC-LP, LMFT-LP, LCAT-LP.

EXPERIENCE REQUIRED

Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment. Experience working with formally incarcerated clients and/or working with justice involved clients. Knowledge of violence risk assessments and court procedures are a plus.

DUTIES AND RESPONSIBILITIES
  1. Perform routine team duties, and have lead responsibility for justice involved clients with regards to integrating dual-recovery treatment, family support or vocational/employment training with the tasks of other team members.
  2. Complete any required AOT, parole, probation, or court progress reports and escort consumers to related hearings and appointments
  3. Involve family, significant others, housing providers, defense attorney, and/or community corrections to ensures compliance with regulations and coordination of services.
  4. Assist the ACT team and consumer in the development of service plan goals and objectives that promote reductions in criminogenic risk and prosocial community living
  5. Coordinate with jail and prison reentry coordinators and visit consumers during detention to facilitate their safe and successful reenter
  6. Coordinate, advocate and liaise with probation and parole officers
  7. Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between Forensic ACT and other systems.
  8. Engage in performing assessments, case management, handling crisis intervention, and discharge planning.
  9. Maintains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations.
  10. Provides direct, face-to-face and/or collateral services to recipients either in the Forensic ACT Office or in the community in places and situations where problems arise.
  11. Responsible for periodic on call coverage to be arranged in advance with the Team Leader
  12. Assists recipients in developing appropriate expectations from treatment.
  13. Strengthens the ability of the recipient to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process.
  14. Utilizes and understand the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the Agency
  15. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  16. Complete QI Training during onboarding and participate in QI refresher training as needed.
