Job summary

CN Guidance & Counseling Services seeks a qualified professional to join the FACT Team, delivering community-based case management and coordination of services for individuals with severe mental illness. The role emphasizes data-driven QA, documentation accuracy, and collaboration with multi-disciplinary teams.

You will provide direct services, telehealth as needed, and work to ensure comprehensive, person-centered care plans that reflect client needs and regulatory requirements.

Qualifications

• Bachelor or higher in social work, psychology, rehabilitation, or related field.
• Master's degree preferred; peer counselor certification accepted.
• Knowledge of Medicaid, Medicare, DSS and housing applications is beneficial.

Responsibilities

• Manages medical records and coordinates the management information system.
• Maintains complete case records per standards and policies.
• Leads Quality Improvement, Utilization Review, and Performance Improvement activities.
• Plan and coordinate services in office or field settings to support care.
• Ensure service plans reflect recipient needs and updated clinical documentation.
• Coordinate understanding of Medicaid/Medicare housing and related applications.
• Facilitate data sharing for joint decision-making with clients.
• Complete QI training and participate in refresher sessions.
• Promote an inclusive culture and safe working environment.
• Provide direct services (in person, telehealth, or collateral).
• Perform other duties as assigned.

JOB DEFINITION:

The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. The Forensic ACT Team provides an integrated set of evidenced-based treatment and support services through a highly individualized approach. Services are provided in the community in places and situations where problems arise.

EDUCATIONAL REQUIREMENTS:

Bachelor, master's degree or higher, in social work, psychology, rehabilitation counseling, or related field preferred. Peer counselor certification is also accepted.

ADDITIONAL REQUIREMENTS:

• Current New York State Driver's License
• Clean DMV Record
• Proof of Motor Vehicle Insurance

EXPERIENCE REQUIRED:

One year experience working in a mental health setting at the masters or higher level or two years' experience working in a mental health setting at the bachelor's level preferred.

DUTIES AND RESPONSIBILITIES:

1. Manages medical records; operates and coordinates the management information system.
2. Ensures that a complete case record is maintained for all recipients in accordance with recognized and acceptable principles of record keeping.
3. Responsible for Quality Improvement, Utilization Review, and Performance Improvement activities including but not limited to data collection, analysis of outcome trends, and identification of service provision, which needs improvement, completion of case records, and the timely submission of required documentation.
4. Plan and coordinate all services, in the office or in the field, which support and supplement the rendering of case management and care coordination services to recipients.
5. Ensure that service plans reflect the recipient's needs. Reviews clinical documents to ensure that revisions to service plans and/or other clinical documentation are done in a timely manner, using appropriate clinical language and properly reflect any changes in recipient's' needs.
6. Must have an understanding of concrete services, i.e., Medicaid, Medicare, DSS and SPOA housing applications.
7. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
8. Complete QI Training during onboarding and participate in QI refresher training as needed.

12. Provides direct, face-to-face, Telehealth and/or collateral services to recipients either in the FACT Office or in the community in places and situations where problems arise.
13. Other duties as assigned.
