Job summary

Community Horizons in De Pere, WI is seeking a compassionate Home Health Aide to support a client at home with daily living tasks and mobility. You will provide personal care, meal prep, light housekeeping, and assist with safe transfers using a Hoyer lift, with training provided.

A valid driver’s license and reliable vehicle are required; you must be 18+ and able to pass a Wisconsin background check. This role offers part-time hours with local travel and a pet-friendly home environment.

Responsibilities

• Provide personalized care including wake ups, evening routines, bathroom assistance, bathing, and medication management.
• Assist with meal prep and light housekeeping (dishes, laundry, sweeping, mopping, dusting).
• Support physical wellness with range-of-motion exercises, repositioning, and safe transfers using a Hoyer lift; training provided as needed.
• Take client on errands or outings using the provided vehicle.

Job description

Pay: $21 /hour

Location: De Pere

Shifts: Every other Sunday 10:00am-10:00pm

What You'll Be Doing:

Instead of balancing a dozen patients at once, your entire focus will be on helping her thrive independently.

• Personalized Care: Assist with later-morning wake ups, evening routines, bathroom assistance, bathing/showering, and medication
• Home & Comfort: Keeping her space comfortable by helping with meal prep and light housekeeping (dishes,laundry, sweeping, mopping, dusting. Etc.).
• Physical Wellness: Support her with range-of-motion exercises, repositioning, and safe transfers using a Hoyer lift. Previous experience with mobility limitations is fantastic but we are fully ready to train the right person!
• Going Out: Taking her on errands or outings using the provided vehicle.
