Job summary

Jobtailor is seeking a dedicated caregiver coordinator to assess client needs and determine eligibility, and to develop service plans for seniors with age-related health conditions. The role includes coordinating non-medical homemaking services and delivering care in a culturally safe manner, with travel to communities as required.

Experience with Indigenous communities, health assessments, and trauma-informed care is essential, along with CPR/AED certification and clear background checks.

Qualifications

  • Minimum post-secondary education, preferably in Indigenous Studies, or a diploma related to Personal Support Work with a minimum 5 years’ experience.
  • Experience delivering services for aging populations.
  • Authentic understanding of Indigenous history, culture, traditions, ceremonies and values.
  • Ability to deliver trauma-informed care and continue learning.
  • Valid CPR and AED Certificates required.
  • Vulnerable Sector Check and a clear TST result.
  • Access to a personal vehicle for regional travel.

Job description

• Assess client needs and determine client eligibility
• Develop plans of service with clients
• Coordinate non-medical homemaking services for individuals aged 55 and up with age-related health conditions, disabilities and/or respite needs
• Deliver services in a culturally safe and appropriate manner
• Work within the Homemaking Framework
• Participate in organizational Indigenous Cultural Safety training and ongoing reflexive professional development
• Travel regularly to meetings and communities
• Attend BANAC clinics, offices and community settings
• Work occasional overtime as required

Requirements

  • Minimum post-secondary education, preferably in Indigenous Studies, or a diploma related to Personal Support Work with a minimum 5 years’ experience
  • A combination of skills, education and life learning specific to cultural knowledge may be considered
  • Experience facilitating and delivering services for aging populations
  • Authentic understanding, practice and ability to share Indigenous history, culture, traditions, ceremonies and values
  • Ability to work according to Indigenous core values of respect and kindness
  • Ability to deliver trauma-informed care and willingness to continue learning
  • Experience completing health assessments using the RH-CHA tool is an asset
  • Experience working with senior and aging populations with age-related conditions and/or disabilities
  • Experience working with Indigenous communities
  • Ability to attend all BANAC worksite locations
  • Valid C.P.R. and A.E.D. Certificates required
  • Valid Class G Ontario Driver’s License required
  • Up-to-date clear Vulnerable Sector Check required
  • Up-to-date clear TST result required
  • Access to a personal vehicle and reliable vehicle required for regional travel
