Job summary

1st Choice & in Morrisania, NY seeks a Registered Nurse UAS Assessor to perform in-home and telehealth NY UAS assessments. You’ll control your five-day workweek with flexible start times, balancing professional impact with personal life in a modern hybrid nursing role.

Your duties include reviewing documentation, communicating with providers, and ensuring NYS-compliant, high-quality notes. A car, NY RN license, and BSN are required.

Qualifications

• Must have own car for daily travel between assessment locations.
• Public transportation and ridesharing are not permitted.
• 1–2 years of RN experience in one-on-one in-home, community, skilled nursing facility, long term and geriatric care.
• RN license valid in New York.
• BSN is required.
• Familiarity with Uniform Assessment System (UAS)-NY is a plus.
• Strong communication, time management, and EHR skills.
• Reliable transportation for in-person visits.

Responsibilities

• Complete UAS NY assessments during in home visits or via telehealth.
• Review medical documentation and record accurate detailed findings.
• Educate consumers and communicate with providers when needed.
• Maintain timely high quality documentation that meets all NYS standards.
• Manage your schedule, route planning, and daily workflow.
• Follow all conflict free requirements under the NYS Department of Health.
