Job summary

1st Choice is hiring a Registered Nurse UAS Assessor for Western NY, New York. This role offers an independent schedule with hybrid in-home and telehealth assessments, allowing you to balance clinical impact with personal flexibility.

You will review medical documentation, educate consumers, and document findings that meet NYS standards. RN license in NY and a BSN are required, with 1–2 years of home health or geriatric RN experience, plus ability to travel using your own vehicle.

Qualifications

• 1–2 years RN experience in home health or geriatric care.
• New York RN license and BSN required.
• Must have own car for travel between assessment locations.
• Familiarity with UAS-NY is a plus.

Responsibilities

• Complete UAS NY assessments during in home visits or via telehealth.
• Review medical documentation and record accurate detailed findings.
• Educate consumers and communicate with providers when needed.
• Maintain timely high quality documentation that meets all NYS standards.
• Manage your schedule route planning and daily workflow.
• Follow all conflict free requirements under the NYS Department of Health.
