Job summary

1st Choice & is seeking Registered Nurses to serve as UAS Assessors in a hybrid model across New York. You will perform in-home and telehealth assessments, making clinical judgments that impact patient care while navigating a flexible workweek.

Ideal candidates hold NY RN licensure with a BSN and have 1–2 years of home health experience. You will use UAS-NY and EHR tools, balancing travel with patient scheduling and ongoing training. Mileage reimbursement is provided.

Qualifications

• Must have active RN license in New York.
• BSN required.
• 1–2 years of RN home health or in-home care experience.
• Familiarity with UAS-NY is a plus.

Responsibilities

• Complete UAS NY assessments during home visits or telehealth.
• Review medical documentation and document findings accurately.
• Educate consumers and coordinate with providers when needed.
• Maintain timely, high-quality documentation that meets NYS standards.
• Manage your schedule, route planning, and daily workflow.
• Follow all conflict-free requirements under NYS Department of Health.
