Job summary

1st Choice & hires a Registered Nurse UAS Assessor for Washington Heights/Inwood, NY. You will conduct UAS NY assessments in-home or via telehealth, review medical records, and document findings to support safe home-based care.

This hybrid role offers a flexible five-day week, start-time choice, mileage reimbursement, and ongoing clinical leadership training. An RN with NY license and BSN is preferred, with 1–2 years in-home care and strong EHR skills encouraged to apply.

Qualifications

• Active RN license in New York.
• 1–2 years of RN experience in one-on-one in-home, community, skilled nursing, long-term and geriatric care.

Responsibilities

• Complete UAS NY assessments during in-home visits or via telehealth.
• Review medical documentation and record accurate, detailed findings.
• Educate consumers and communicate with providers when needed.
• Maintain timely, high-quality documentation meeting NYS standards.
• Manage your schedule, route planning, and daily workflow.
• Follow all conflict-free requirements under NYS DOH.
