Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to provide advanced, home-based palliative care and lead patients and families through the treatment journey.

You will assess, plan, and manage complex cases in collaboration with a multidisciplinary team, while ensuring Medicare/Medicaid compliance and high-quality documentation. This full-time role offers competitive pay, full benefits, and opportunities for professional growth.

Formación

• Master of Science in Nursing required.
• Minimum two years NP experience.
• Family or Adult medicine NP preferred.
• Experience with Part B billing preferred.
• Ability to work autonomously in an advanced practice role.

Responsabilidades

• Deliver home-based, patient-centered palliative care in collaboration with the care team.
• Perform comprehensive patient assessments including physical, emotional, and psychosocial aspects.
• Develop and implement individualized care plans with patients and families.
• Adjust treatment plans based on patient needs and progress.
• Prescribe medications and order diagnostics as allowed by state law.
• Maintain timely, accurate clinical documentation per professional standards.
• Communicate effectively with physicians, caregivers, and other healthcare professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
