NEMS is seeking a PACE Physical Therapist in San Jose to deliver autonomous PT services as part of the PACE interdisciplinary team. This full-time role includes a $20,000 sign-on bonus and a salary range of $112,247.20 to $128,752.00 annually, with responsibilities across assessments, care planning, and home health visits.

The position emphasizes collaboration with the IDT, patient education, and adherence to PACE standards. Experience with elderly populations and Epic is a plus.

Competenze

• Bachelor, Master or Doctoral degree approved by the California Board of Physical Therapy.
• Licensed to practice as a physical therapist in California.
• Current BLS and First Aid required.
• Fluent in English; Chinese (Cantonese) preferred.

Mansioni

• Conducts initial and periodic physical therapy assessments to determine care needs, preferences and goals.
• Develops and implements rehabilitation program to promote, restore or maintain functioning.
• Supervises rehab therapy assistants and participates in IDT.
• Provides home visits and home health as needed.
• Utilizes Epic for documentation and workflow, serving as content/workflow expert.
