Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier home health care in the Herald area. This independent contractor role focuses on adults of all ages and requires assessment, planning, and execution of OT services in patients’ homes.

You will determine treatment plans, perform interventions, and monitor progress, while collaborating with families and care teams. The position offers flexible scheduling and independent practice in a locally focused setting.

Qualifications

• Licensed and registered Occupational Therapist in the state.
• Completion of an accredited OT program.
• Ability and enthusiasm to work with all ages.

Responsibilities

• Assess patient care needs related to functionality, ADLs, fine motor, home assessments, and adaptive equipment.
• Develop and implement treatment plans to improve wellbeing.
• Perform interventions and monitor patient progress, adjusting care as needed.
• Coordinate with the health care team and communicate with patients and families.
• Lead patients in therapy activities to improve functioning and safety.
