Job summary

FeldCare Connects in North Port, FL seeks a self-motivated Occupational Therapist to deliver premier care to adults in home health. As an independent contractor, you will assess patients' functional status, plan therapy, and coordinate with the care team to support daily living activities.

Qualifications

• Must hold a state-licensed Occupational Therapist with active registration.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Assess patient care needs related to ADLs, functionality, and home safety.
• Develop and implement treatment plans and monitor progress.
• Coordinate care with the health care team and document outcomes.
• Educate patients and caregivers on therapy activities and home programs.
