Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license.
• MD/DO ophthalmology degree and license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form.

Responsibilities

• Lease and operate an independent optometry practice inside Walmart stores.
• Establish your days of practice and hours of operation.
• Set your patient schedule and fee schedule.
• Provide eye care and general health assessments to patients.
