Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead patient care related to eye and vision health, as well as general well-being.

Responsibilities include:

• Launching your services in Walmart stores
• Establishing your practice days and hours
• Managing your patient schedule
• Setting your fee schedule
• Retaining 100% of exam and professional fees

Optional support services (at fair market value) include:

• Equipment and instrumentation
• Administrative and/or Opt-Tech support services (if permitted by state)

Minimum qualifications for lease agreement:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements:

• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
