Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead patient care related to eye and vision health, as well as general well-being. You have the autonomy to:

1. Establish your practice days and hours
2. Control your patient schedule
3. Set your fees
4. Keep 100% of exam/professional fees

Optional support services include equipment/instrumentation and administrative/opt-tech support, subject to state regulations, for fair market value.

Minimum qualifications include:

1. Doctor of Optometry (OD) from an accredited school with a valid state license
2. Ophthalmologist (MD or DO) from an accredited school with a valid state license
3. General Liability and Professional Liability Insurance Certificates
4. W-9 form
