Position Summary...

What you'll do...

Launch your services in Walmart stores! As an Independent Optometrist leasing space inside our Walmart stores, you will play a leading role in patient care related to eye and vision health, as well as overall well-being.

As an independent leaseholder, you have the autonomy to:

1. Establish your days of practice
2. Set your hours of operation
3. Control your patient schedule
4. Determine your fee schedule
5. Keep 100% of exam/professional fees

For a fair market value, you can opt into:

1. Equipment/instrumentation
2. Administrative and/or Opt-Tech support services (if permitted by state)

Minimum Qualifications for a lease agreement:

1. Doctor of Optometry (OD) from an accredited optometry school
   • Valid state optometry license
2. Ophthalmologist (MD or DO) from an accredited school
   • Valid state ophthalmology license
3. General Liability Certificate
4. Professional Liability Insurance Certificate
5. W-9 form

ME & VA license agreements are managed through Gumberg Asset Management Company.
