Launch your services in Walmart stores as an Independent Optometrist leasing space inside our stores. You will play a key role in patient care related to eye and vision health, as well as general well-being.

Autonomy as a Leaseholder

As an independent leaseholder, you can:

• Set your practice days and hours
• Manage your patient schedule
• Determine your fee structure
• Keep 100% of exam and professional fees

Optional support services available at fair market value include equipment/instrumentation and administrative/opt-tech support (if permitted by state).

Minimum Qualifications

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements include:

• Ophthalmologist (MD or DO) with valid state license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form

ME & VA license agreements are managed by Gumberg Asset Management Company.
