Minimum Qualifications

• Doctor of Optometry (OD) from an accredited optometry school with a valid state license
• Ophthalmologist (MD or DO) from an accredited school with a valid state license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
