Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead patient care related to eye and vision health, as well as general well-being. You will have the autonomy to:

1. Establish your practice days and hours
2. Control your patient schedule
3. Set your fee schedule
4. Keep 100% of exam and professional fees

You can also opt into additional services such as:

• Equipment and instrumentation
• Administrative and Opt-Tech support services (if permitted by state)

Minimum Qualifications for lease agreement:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
