Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will play a key role in patient care related to eye and vision health, as well as general health and well-being.

As an independent leaseholder, you have the autonomy to:

1. Establish your practice days
2. Set your hours of operation
3. Manage your patient schedule
4. Determine your fee schedule
5. Keep 100% of exam/professional fees

Additional options available at fair market value include:

1. Equipment and instrumentation
2. Administrative and/or Opt-Tech support services (if permitted by state)

Minimum qualifications for a lease agreement:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
