Position Summary

Launch your services in Sam's Club! As an Independent Optometrist leasing space inside our stores, you will take a leading role in patient care related to eye and vision health, as well as general wellbeing.

As an independent leaseholder, you have the autonomy to:

• Establish your practice days
• Set your hours of operation
• Control your patient schedule
• Determine your fee schedule
• Keep 100% of exam/professional fees

For a fair market value, you can opt into:

• Advanced equipment/instrumentation
• Administrative and/or Opt-Tech support services, if permitted by state law

Minimum qualifications for a lease agreement include:

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist MD or DO from an accredited school
• Valid state ophthalmology license
• General Liability and Professional Liability Insurance Certificates

Note: ME/VA license agreements are managed through Gumberg Asset Management Company.
