Position Summary... Independent Optometrist – Sam’s Club

Launch your services in Sam’s Clubs! As an independent lease holder, you will take a leading role in patient care related to eye and vision health, as well as general well-being.

You will have the autonomy to:

1. Establish your days of practice
2. Set your hours of operation
3. Control your patient schedule
4. Set your fee schedule
5. Keep 100% of exam/professional fees

At fair market value, you can opt into:

1. Equipment/instrumentation
2. Administrative and/or Opt-Tech support services (if allowed by state)

Minimum qualifications for a lease agreement include:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability and Professional Liability Insurance Certificates
• W-9 form
