Position Summary

Launch your services in Sam's Club! As an Independent Optometrist leasing space inside our stores, you will play a leading role in patient care related to eye and vision health, as well as general wellbeing.

As an independent leaseholder, you have the autonomy to:

• Establish your practice days
• Set your hours of operation
• Control your patient schedule
• Determine your fee schedule
• Retain 100% of exam/professional fees

Optional enhancements at fair market value include:

• Advanced equipment and instrumentation
• Administrative and/or Opt-Tech support services, if permitted by state law

Minimum Qualifications

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license
• Ophthalmologist MD or DO from an accredited school
• Valid state ophthalmology license
• General Liability and Professional Liability Insurance Certificates
