Walmart invites an Independent Optometrist to lease space inside Sam’s Club in Lexington, KY. You will lead patient care in eye and vision health while enjoying autonomy over practice days and hours, patient scheduling, and fee setting. Retain 100% of exam fees and optionally access equipment and administrative support if allowed by state law.

As the lease holder, you will be responsible for establishing a schedule, fee structure, and delivering high-quality eye care within a Walmart environment.

Qualifications

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate/Professional Liability Insurance Certificate.
• W-9.

Responsibilities

• Launch your services in Sam's Club and take a leading role in eye and vision care.
• Establish your days of practice and hours of operation with autonomy.
• Control your patient schedule and set your fee schedule.
• Keep 100% of exam/professional fees.
• Opt into equipment/instrumentation and admin/tech support if state allows.
