Formación

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid General Liability Certificate
• Professional Liability Insurance Certificate
• W-9

Responsabilidades

• Independent Optometrist leasing space inside Sam’s Club and leading patient care in eye and vision health.
• Establish your days of practice and hours of operation; control your patient schedule.
• Set your fee schedule and keep 100% of exam/professional fees.
• Opt into equipment/instrumentation and administrative/OP tech support where allowed by state law.
