Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school and valid license.
• General/Professional liability insurance certificate.

Responsibilities

• Launch your optometry services in Sam's Club and lead patient eye care and general wellbeing.
• Establish your days of practice and hours of operation; control your patient schedule.
• Set your fee schedule and retain 100% of exam/professional fees.
• Optionally access advanced equipment and/or support services where state law allows.

Valid state optometry license
