Qualifications

• OD from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9.

Responsibilities

• Lease independent space inside Sam’s Club and provide eye/vision care.
• Set days of practice and hours of operation.
• Control patient schedule and set fee schedule.
• Keep 100% of exam/professional fees.
• Opt into equipment/instrumentation and/or administrative support if allowed.
