FeldCare Connects is seeking a self-motivated Occupational Therapist for the Moorpark area in a home health Independent Contractor role. You will assess daily living activities, plan and implement therapy to improve function, and educate patients and families in safety and self-care.

Flexible schedule and autonomy are emphasized in this position, with support for coordination and quality assurance. Experience in adult home health preferred; state OT license required; bilingual skills a plus.

Qualifications

• Licensed OT with state registration required.
• Ability to work with patients of all ages.
• Completion of an accredited OT program.
• Bilingual skills are a plus.

Responsabilités

• Assess patients' functional status and daily living activities.
• Develop and implement an OT treatment plan.
• Lead patients in therapeutic activities to improve function and safety.
• Educate patients, families, and caregivers on therapy and home safety.
