Job summary

Northeast Healthcare Recruitment seeks a Nurse Practitioner to perform in-home Health Risk Assessments for Medicare Advantage patients across three Ohio counties. You’ll review medical histories, assess health needs, and identify care gaps during each visit.

This is a 1099 independent contractor role with flexible hours. Ideal candidates are board-certified NPs licensed in Ohio with Home Health experience and the ability to manage own schedule while meeting coverage demands and reimbursement

Qualifications

• Active Ohio NP license and board certification required.
• Experience conducting in-home health assessments for Medicare Advantage.
• Willingness to cover 3 Ohio counties and travel as needed.

Responsibilities

• Conduct Health Risk Assessments for Medicare Advantage patients in home settings.
• Document findings and coordinate care gaps to improve outcomes.
• Manage own schedule as a 1099 independent contractor.
