Job summary

COMMUNITY CARE HOME INC seeks a Direct Support Professional to provide personal care, supervision, and daily living support in a residential setting. You will implement each Individual’s Support Plan (ISP), document progress, and collaborate with families and other providers.

The role emphasizes safeguarding health and wellbeing, promoting independence, and helping residents engage with community activities and outings.

Qualifications

• High school diploma or GED.
• Professional License if applicable.
• Pass State PCA Competency Test if diploma unavailable.
• Knowledge, skills and abilities to perform the functions of this position.

Responsibilities

• Provides personal care and protective oversight and supervision.
• Provides services, supports, care and treatment and/or supervision as required.
• Participates in the development and implementation of each Individual’s Support Plan (ISP).
• Provides supervision as determined in the ISP.
• Adheres to applicable regulatory requirements.
• Responsible for personal and financial transactions as assigned.
• Ensures facility/home is safe, comfortable, and hazard-free.
• Provides coverage in emergencies.
• Performs other duties as assigned.
• Assesses and documents instructional needs and progress.
• Ensures health, safety, welfare, and community inclusion for individuals.
• Assists with self-help, socialization, and adaptive skills training.
• Implements behavioral support plans to reduce maladaptive behaviors and teach alternatives.
• Assists with independent living skills including personal care, light housework, transportation, etc.
• Teaches rule compliance, attendance, task completion, problem solving, endurance, and safety.
