Job summary

Communitas, Inc. in Wakefield, MA is seeking an Independent Living Skills (ILS) Mentor to empower individuals with developmental disabilities. You will guide them through ISP goals, support daily living activities, and advocate for their needs in home and community settings.

Responsibilities include supervising health and welfare, assisting with assessments, and collaborating with families to optimize outcomes. A HS diploma and driver’s license are required.

Qualifications

  • High School diploma required (BA degree in Human Services or related field preferred).
  • At least 1 year of experience supporting adults with developmental disabilities is required.
  • 3 years of experience preferred.
  • A passion and dedication to supporting our people is a must.
  • Valid Driver’s License, reliable transportation, and acceptable driving record.
  • Must pass company background and reference checks.

  • Empower individuals in managing and exercising control over their ISP.
  • Directly work with individuals on their ISP goals.
  • Assist individuals with activities of daily living when necessary.
  • Seek input from individuals and families; and respond accordingly.
  • Help individuals to advocate for their needs and desires; recognize rights to make decisions and take risks.
  • Supervise overall medical health, nutrition, social and general welfare.
  • Assist in the completion of assessments and other paperwork as assigned.

Education

The role!

Position: Independent Living Support (ILS) Mentor

Location: Wakefield, MA

Pay rate: $20 per hour

Hours: 10 – 25 hours per week

Shifts: Morning, afternoon, evening, and weekend hours available

Summary & Responsibilities

IHS Mentors are responsible for teaching life and social skills to individuals in a variety of environments using their home and community as the classroom.

  • Empower individuals in managing and exercising control over their Individual Support Plan (ISP)
