Job summary

Communitas, Inc. is seeking an Independent Living Support (ILS) Mentor in Wakefield, MA. The role offers 10–25 hours per week at a pay rate of $20 per hour, with morning, afternoon, evening, and weekend shifts available.

The position emphasizes teaching life and social skills, supporting ISP goals, and assisting with daily living activities. A valid driver’s license and a commitment to empowering individuals are required.

Qualifications

• High school diploma required; BA degree in Human Services or related field preferred.
• At least one year of experience supporting adults with developmental disabilities is required; three years preferred.
• Passion and dedication to supporting our people are a must.
• Valid driver's license, reliable transportation, and acceptable driving record.
• Eligibility: legally authorized to work in the United States; at least 21 years old; current valid driver’s license; minimum high school diploma or equivalent; at least one year of experience supporting people with disabilities.

Responsibilities

• Teach life and social skills in home and community settings.
• Support individuals in managing and following their ISP goals; assist with ISP activities.
• Assist with activities of daily living as needed.
• Seek input from individuals and families; respond accordingly.
• Help individuals advocate for their needs and rights; support decision-making and risk-taking as appropriate.
• Supervise overall health, nutrition, social and general welfare; complete assessments and paperwork as assigned.

Responsibilities

• Teach life and social skills to individuals in a variety of environments using the home and community as the classroom
• Empower individuals to manage and exercise control over their Individual Support Plan (ISP)
  • Directly work with individuals on their ISP goals
• Assist individuals with activities of daily living when necessary
• Seek input from individuals and families; respond accordingly
• Help individuals advocate for their needs and desires when necessary
  • Recognize and support the individual’s rights to make decisions, choices, and the right to take risks
• Supervise overall medical health, nutrition, social and general welfare
• Assist in completing assessments and other paperwork as assigned
