Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License with acceptable driving record is required.

Responsibilities

• Develop individualized, person-centered service plans and implement them.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources as needed.
• Implement activities that promote community inclusion, relationships, personal growth, and valued social roles.
• Maintain a safe, clean, and supportive environment for individuals served.
• Review, complete, and maintain all required documentation in a timely manner.
