Job summary

FOX Rehabilitation is seeking licensed Speech Language Pathologists to deliver one-on-one care to older adults in private residences or Senior Living Communities in Panora, IA. You will design and implement individualized plans, perform assessments, and use evidence-based treatments within a flexible schedule.

Therapy may include telehealth options and collaboration with an interdisciplinary team to promote safety, function, and aging in place across diverse settings.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with a PT/OT/SLP/Fitness team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
