FOX Rehabilitation is seeking licensed Speech-Language Pathologists for in-home and senior living settings in Decorah, IA, and surrounding areas. You will deliver one-on-one care to older adults, conduct evaluations, develop plans, and provide evidence-based treatment within a flexible schedule.

Telehealth and virtual care options help extend access and continuity of care, while collaboration with an interdisciplinary team supports comprehensive patient outcomes and professional growth.

Qualifications

• Licensed Speech-Language Pathologist required in the state(s) of practice or eligibility to obtain license.
• Degree from an accredited Speech-Language Pathology program.
• Current CPR certification and CCC-ASHA (preferred).

Responsabilités

• Provide one-on-one Speech Language Pathology services to older adults in-home or within senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility and independence.
• Document effectively within FOX’s EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
