Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier care in the Aptos area as an Independent Contractor. You will assess patient needs, set a plan, provide home health PT, and monitor progress to promote movement and reduce pain.

We value independence, flexible scheduling, and maintaining accurate records while delivering high-quality care to adults in home settings.

Qualifications

• Licensed Physical Therapist in the state.
• Experience applying PT principles in rehabilitation.
• Bilingual is a plus.

Responsibilities

• Assess and evaluate patient care needs to promote movement, reduce pain, restore function, and prevent disability.
• Determine a treatment plan based on assessment and evaluation, implement interventions, and monitor progress.
• Deliver interventions to improve patient well-being and monitor patient progress.
