Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier care for adults in the San Antonio area as an Independent Contractor. The PT evaluates patient needs, creates and implements treatment plans, and monitors progress to improve mobility and reduce pain in home health settings.

Qualified candidates must hold a PT license and state registration, complete an APA-approved curriculum, and possess strong communication and record-keeping skills.

Qualifications

• Remarkable knowledge of physical therapy principles and methods in rehabilitation.
• Ability to provide a variety of treatments with precision and care.
• Excellent written and oral communication skills.
• Understanding of therapy's role based on patient age, needs and environment.
• Accurate and discreet maintenance of patient records.
• Sound clinical reasoning in treatment planning and progress monitoring.
• Ability to modify protocols and report changes to the supervising therapist.

Responsibilities

• Assess and evaluate patient care needs to promote movement, reduce pain, restore function, and prevent disability.
• Develop and execute a treatment plan and monitor progress in home health.
• Document assessments and interventions clearly and timely.
